Occurrence of an Accident/Incident The accidents and Incidents subject to investigation by the commission are: (1) the crash,collision,or fire of an aircraft;(2)Death of or injury to persons,damage or destruction to property caused by an aircraft;(3)Death(excluding death by natural causes)or disappearance of persons on board an aircraft;(4)A collision with another aircraft;(5)Serious incidents(situations which recognized to carry the risk of an accident). Notification of Accident/Incident Commencement of Investigation of Accident/Incident Appointment of investigator‐in‐charge and investigator Coordination with police and other relevant organizations Notification to the State of Registory,Operation,Design,Manufacture and International Civil Aviation Organization(ICAO) Fact-finding Investigation,etc. Interviews with crew/witnesses;acquisition of weather report and other relevant documentations Collect flight data recorder(FDR)and cockpit voice recorder(CVR),and investigate the state of damages to aircraft. Notify the preliminary report to ICAO Tests and reseach (analysis,investigation,and experimentation) Analysis Hearings(Parties concerned and persons of learning and experience) Deliberations of the Commission(Air Sub-committee) Hearings(Persons related of the cause) Resolution of the commission(Air Sub-committee) Submission the report to Minister of Land,Infrastructure and Transport Publication of the Report Submission the report to ICAO
