Occurrence of an Accident/Incident The accidents and Incidents subject to investigation by the commission are:(1) Train collision;(2)Train derailments;(3)Train fires;(4)Other accidents(limited to accidents involving the death of a passenger, crew ,etc.,5 or more injuries,or particularly unusual accidents);(5)Serious incidents(situations which are recognized to carry the risk of an accident). Notification of Accident/Incident Commencement of Investigation of Accident/Incident Appointment of investigator‐in‐charge and investigator Coordination with police and other relevant organizations Fact-finding Investigation,etc. Interviews with crew,passengers witnesses,etc.;acquisition of weather report and other relevant documentations Collection of evidence from accident,and investigation of the state of damage to railway facilities and rolling stock. Tests and reseach (analysis,investigation,and experimentation) Analysis Hearings(Parties concerned and persons of learning and experience) Deliberations of the Commission(Railway Sub-committee) Hearings(Persons related of the cause) Resolution of the commission(Railway Sub-committee) Submission the report to Minister of Land,Infrastructure and Transport Publication of the Report
